your code looks alright, it might help to say what the input was, and
what error you got.

of course since you are using a fixed name "a" you don't need
xsl:element here
<xsl:element name="a">
<xsl:attribute name="href">
/servlet/com.commercequest.enet.status.web.StatusQuery?report=Document
Detail&uuid=<xsl:value-of select="@uuid"/>
</xsl:attribute>
could more simply be done using a literal result element
<a href="/servlet/com.commercequest.enet.status.web.StatusQuery?report=Document
Detail&uuid={@uuid}">
<xsl:choose>
...
</xsl:choose>
</a>
