I had the same problem as you with the xml:lang attribute. I believe that it
was a bug in one of the versions of Xalan. I am now running Xalan 1.2 and no
longer see the problem, although my xml has not changed.

I would suggest grabbing the latest released version of Xalan and trying that.
Unfortunately I cant help you with the second issue. Perhaps someone else can.
