<xsl:stylesheet xmlns:xsl="http://www.w3.org/XSL/Transform/1.0"
wrong namespace (that was used in one of the early drafts but wasn't the
final choice)
it should be http://www.w3.org/1999/XSL/Transform
xmlns="http://www.w3.org/TR/REC-html40"
If you are writing html you probably don't want to use any namespace
so I would delete the above line. (It was required to turn on html
output method in some early drafts but not in the final version)
result-ns=""
this attribute was removed from the drafts, so it is now an error.
You also have to have a version attribute,
version="1.0"

It seems that you have some very old documentation on XSL syntax.
Make sure you have the 1.0 REC version of the specification.
