Updates to the Contribution Checklist.

Added the following changes to the contribution checklist:
1. One patch per email.
Made mention that it must be one patch per email and that
patchsets need to go out as multiple emails. This facilitates
patchwork workflow and makes it easy to track single patch
updates. Note that patchwork does not support > 1 patch per email.
Ran into this problem while reviewing Mike Fabian's Unicode 8
patchset (5 patches in one email).
2. Use "[PATCH vN N/M] <Subject>"
More closely match lkml subject lines for versioned multipatch
patchsets. Keeping in mind the comments about subjects and
patchsets.
3. Accept "<Subject> [committed]" or "[COMMITTED] <Subject>"
Patches from subsystem maintainers going straight in can be
sent as above.
