move the continuations support code into its own file


Something I had proposed myself doing a couple of years
ago ...

I've applied this patch to move the continuations code
out of utils.c into its own file (and no longer implement
them on top of cleanups).  A bit of simplification will follow.

Tested on x86_64-linux, async.
