[rfc/rft] Fix incorrect static types in mdebugread.c


Hello,

mdebugread.c allocates a static set of types in its _initialize_mdebugread,
depending on current_gdbarch settings during startup.  That's broken in a
multi-arch setup (even today with --enable-targets=).

The patch below removes the static types, and holds a per-objfile list of
types representing the bt* basic types instead.

Note that I've added the 64-bit versions of some bt* flags to the
include/coff/symconst.h header file.  These are based on the names in
comments in the original source code, and also seem to correspond to
what the documentation says e.g. here:

(Except that btLongLong and btULongLong have different values in symconst.h
today.  I didn't change those, but instead added values corresponding to
the documentation as btLongLong64 and btULongLong64, as was indicated by
comments in the old mdebugread.c code.  This doesn't change behaviour of
existing code -- but may actually be wrong.  I'd appreciate any comments ...)

This patch should not change the behaviour in a single-arch setup, but
I'm unable to test as I don't have any machine using the format.
