[cplus] Output tweaks


A grab-bag of tweaks.  The only things left in my current testsuite:
  - A few parse errors caused by conversion operators to function pointer
types.  This is the typeof issue.
  - A few parse errors caused by differences in the handling of __GLOBAL_. 
This is unimportant.
  - A large number of parse errors caused by G++'s tendency to emit mangled
names referencing function-scope variables like  "foo()::__PRETTY_FUNCTION__".
This is important and next.
