(lookup*) documentation patch

Hi,

As per J. Lamour's request on -discuss, I am attaching a patch to
io.sgml that documents the (*lookup) callback.  I agree that a bit of
glue might be needed to smoothly integrate this, but I'll leave that
to whoever has the 'master plan' for these docs in their head.

Looks like the docs could use some cleanup in general... SGML->XML,
and possible changing some of the markup around to better utilize
docbook (funcsynopsis, for instance).  But that's a big job for
someone who's making money on all this:-)
