| When rendering this document with any of the HTML or DVI backends, the
| footnote labels are doubled in both cases. You'll get a doubled
| superscript number with DVI and a doubled square-bracketed number with
| HTML.
Remove the <footnoteref>. A footnote reference is automatically
generated at the location of the footnote.
