All the FreeBSD Documentation is in DocBook, and issues like table of
contents generation are all handled by the stylesheets.

The SGML source code for these is in the FreeBSD CVS repository. The
Handbook documents various ways you can get hold of this, but probably the
simplest way is over the web -- take a look at
http://www.FreeBSD.org/cgi/cvsweb.cgi/doc/
