Could it be possible that mergeing duplicate page numbers removes the page number prefixes? If so, how could this be fixed?

Probably yes as page number generated by <fo:page-number/> can not be compounded AFAIK. As a workaround you can switch off xep.extensions and remove conditions testing xep.extensions in fo/docbook.xsl from where bookmark code is invoked.
