Hi Paul, you wouldn't mind putting the sample you've collected online somewhere would you? I'm sure your query will be repeated many times in the future by new adopters. In addition to that, the only thing holding back some authors (such as myself) is imagination - I dare say there are many ways of making docbook output more useful/attractive, but I just haven't thought of them or seen them.
