Can you peek in your fo file with a text editor to see
if the reference to the graphic is in there with
the right path?
If so, then perhaps the problem is one of relative
paths. It looks like you are passing to FOP a relative
path to your fo file. Perhaps the relative path
of the graphic doesn't add up. Do you get any
error message about graphic file not found?
