Problem with symbol section of index in HTML output

Hello,
Using DocBook XML 4.1.2. / XSL 1.50.0 stylesheets with Saxon 6.5.1 on
Windows 2000.
I have a problem with the generation of the 'symbol' section of the index.
The rest of the index seems to be ok.
Only a few indexterms show up in the symbol section (maybe 10 out of 500).
Sample:

<title>General</title>
- <indexterm>
<primary>00250</primary>
<secondary>General</secondary>
</indexterm>
- <indexterm>
<primary>General</primary>
<secondary>00250</secondary>
</indexterm>
- <indexterm>
<primary>General</primary>
<secondary>Products Interface</secondary>
</indexterm>
- <indexterm>
<primary>Products Interface</primary>
<secondary>General</secondary>
</indexterm>
I do this programmatically for each of the 500 functions in total.
Then the index looks like:
Index
Symbols
00250
Browser, Browser
General, General
Menu Path, Menu Path
Messages, Messages
Viewer, Viewer
11000
Browser, Browser
General, General
Menu Path, Menu Path
Messages, Messages
Viewer, Viewer
etc.
Which looks ok but has missing indexterms. I have numerous other symbols
between 00250 and 11000 that are in the XML file but not in the HTML output.
Any of you know what could be wrong?
