I added the following to my fo customizations and I now get bookmarks:
<xsl:param name="fop.extensions" select="1"/>
However top-level chapter bookmarks don't work. They appear in Acrobat but clicking on them doesn't cause a redisplay. Tapping on an indented section heading does cause a correct re-display.
