ah well if you want high quality print then I have to put on my other
hat and say use (La)TeX (which is what you suggested originally)
there's several people working on mathml to tex conversion
(using xslt, mainly) but perhaps it's simpler to go straight from
arbortext's markup to TeX. Of course arbortext began life as "TeX at Ann
Arbor" and will be doing just that conversion internally I'd guess (I
don't have a copy)
If you're feeling brave xmltex (ask google) can use tex to directly
typeset mathml (and FO, with the passivetex files)
