Increasing the level of verbosity generates no error/warnings in Passes 1-4 and no additional output in the failing Pass 5.

I haven't seen this before, but you might try looking at the output of dmesg or looking in /var/log/messages (depending on how your system is configured) for a better error message from the equivalent of insmod. Hopefully you'll get the name of the unknown symbol. If that doesn't work, try running insmod yourself on the cached module and see what you get.
