Remove __ASSUME_POSIX_CPU_TIMERS


The macro __ASSUME_POSIX_CPU_TIMERS is defined unconditionally for all
supported Linux kernel versions, and used only in Linux-specific code.

This patch removes this macro (along with nearby related __NR_*
conditionals for associated syscalls that are available everywhere in
2.6.16 and later) since it is now unnecessary, so completing the
__ASSUME_* conditionals removals made possible by using 2.6.16 as
earliest supported kernel version.  (I'm sure there are various __NR_*
conditionals that can still be removed - there, what's relevant is the
minimum supported kernel header version, 2.6.19.1, remembering that
some such conditionals might be relevant on asm-generic architectures
with their smaller set of syscalls but not otherwise.)

Tested x86.  A version diffed with -w is attached for review
convenience.
