#4057 is apparently a result of the combination of
requestDeleteInstructionObserver() + requestUnblock(instructionObserver).
Can be observed by trying to 'continue' almost any non-debuginfo program in the
sourcewindow.
When working with step-over and step-out in RunState, I did not notice this
bug, but the difference is that in that case the InstructionObserver was being
deleted from CodeObserver.addedTo(), instead of when both were fully blocking
the Task.
