DOCBOOK: Page numbers in printed documents

I apologize if this is asked often, or if the solution is
obvious. Quite frankly, I'm clueless, and the folks who I've asked
haven't been able to help with this. :) Also, I'm not sure what may be
causing this problem -- if it's a docbook issue, a TeX problem,
something with Jade, etc. If this isn't the appropriate list for my
problem, please tell me where I can go for help.

I'm using docbook-xml with Jade and the docbook-stylesheets. I'd like
to convert a docbook-xml document into PDF for printing. I have the
following alias set up to spit out a Jadetex document using the
docbook-print stylesheets:

alias xml2print jade -d /usr/lib/sgml/stylesheet/dsssl/docbook/nwalsh/print/docbook.dsl -t tex /usr/lib/sgml/declaration/xml.dcl $1

I then run xml2print on my document, exi.xml. This produces a jadetex
document, and I run pdfjadetex on that. This produces a PDF document,
but the table of contents lines look like:

The only problem is the ??'s instead of page numbers. This has
happened with several versions of the Debian packages for jade,
jadetex, etc. So, I doubt that the problem is a bug in the software; a
bug in Nolan is far more likely. ;)

So, I decided to run jadetex and see if the DVI looked the
same. Running jadetex produces the following:

So, I began to wonder if these errors may have had something to do
