I suspect that the page break is a FOP bug, triggered by the footnote.  What
happens when you remove the footnote?

The indent is hardcoded into the templates with match="biblioentry" and
match="bibliomixed" in fo/bibio.xsl.  Those properties should probably be in
an attribute set, so filing a feature request on the SourceForge site would
be a good future improvement.  For now, you'll have to customize those
templates to remove the indents.
