Are you turning on the double.sided parameter (e.g. <xsl:param
name="double.sided" select="1"/>)?  I'm pretty sure the stylesheets started
the chapter on the right-hand page when this parameter was set.  (I've been
making so many customizations related to pagination that it's hard to
remember how the stylesheets worked out of the box for me.)

I know I saw this somewhere today (have spent the entire day customize
my XSL for DocBook), but now cannot find it.
