I wrote a reference guide for Web and publishing using Docbook:

This is about 60 pages with TOC and index
this was then the basis for:
http://www.blender.nl/showitem.php?id=219
which is produced out of RTF imported into a DTP application and made
a bit nicer.
Then this base was used as reference part to a about 300 pages book
about game creation (to be published). All written and edited (several
Tutorials were written by other authors and were "ported" from HTML or
MSWord by hand) in XEmacs using psgml.
Out of "historical" reasons I use Docbook/SGML, also I have heard that
the RTF (very important here) output is the best so far here.
