Simple example to generate a TOC in a DVI and a PDF file


I have spent several days trying to create a DVI and a PDF file that
contains a Table of Contents (TOC), to no avail.

Does anyone know where I could find very simple example (or instructions) to
create a DVI/PDF that contains a TOC?

The best I've ever done is create a DVI where the table of contents contains
proper page numbers for the first page, but all other page numbers say '???'
or '-999'.  dvi2ps won't even attempt to convert the resultant file,
spitting out a hundred errors.

I have made numerous attempts with several different source files, using the
collateindex.pl script, but still have come up empty handed.

When outputting to HTML, I do get an index, so I know I'm doing something
right.

I have used the phpdoc and Linux Documentation Project files to try to build
on what others have done, but both of these are *way* to complicated.  I
can't get either of these to build.

RedHat 7.0 (and/or Debian 2.2 Potato)
DocBook 4.1
DocBook XML 4.1.2
DSSSL 1.60
OpenJade 1.3
JadeTex 2.20
