But there's no netpbm-config script or the like, so how would a
dependant package know to look for the headers there?
FWIW, debian[1] and gentoo[2] also package netpbm like I did.

Well, -fPIC is the real no-op, -DPIC is just a define and is at the
worst only extraneous; I believe there was some discussion about this a
while ago on cygwin@.
