convert Xtensa files to ISO C90

Now that the Xtensa files have been updated, I can finally update them to use 
ISO C90.  Besides the obvious formatting changes, I also reordered some of the 
functions to reduce the number of prototypes.

Tested with an xtensa-elf target and testsuite runs.  Committed on mainline.
